It's actually a lot easier than you think.
There is a reason why tanks always have to be escorted with at the least infantry, all it would take is one man and some explosives to run up on it and disable it.

Just throw smoke grenades so the tank can't see you. Then pick off the support soldiers and piss of the tank commander. When he pops his head out you can tranquilize him and then extract him and the tank in full condition.
