He had hinted at seeing children being executed and it was implied he had done similarly terrible things himself. He was also the first protagonist to show hesitation the first time he was ordered to kill someone.
Contrast this with all the previous protagonists, who were criminals that primarily harmed other criminals. Rockstar definitely took a new route with IV, experimenting with more serious events and a grim ending the previous entries never attempted.
