You should play through the Yakuza series and realize how badass Dragons and Onis can be. Good luck finding a tattoo artist who can replicate something like this though. Most of them are pretty mediocre now of days.

you find a tattoo artist you like and you discuss your ideas together, then they draw it out for you and if you like it you get it tattooed. I think usually you dont even have to draw out anything for them if you dont want to.

That's correct. You will more than likely have a different art style than your artist anyway, which could even make it more complicated. You can bring references with you though and then go from there.
