It might be a gaping security hole, but not if you don't install a malicious module. AFAIK, there has only been 1-2 malicious modules. Xposed is open source.
You can change almost everything about your ROM or an individual app, whether it be for a useful addition, or purely cosmetic.
See pic related. A module that adds the CM13 visualiser to the notification shade. Completely useless, but why not?
