weight gain is primarily the result of having too high levels of insulin -- which is why sugary foods are often known anecdotal as more 'fattening' than more complex carbs. the insulin then drives whatever calories you intake into body fat, and makes getting rid of it really, really hard. and yes, insulin levels can be elevated by food intake, exercise, but a lot of it is just due to innate factors -- it's why diabetes isn't at a 1;1 correlation with diet/exercise habits. there are absolutely loads of reasons people can develop it, without it being in their control. this is why, say, something that upregulates leptin such as a ketogenic diet, creates fat loss while eating the maintaining the same level of calories

Medical problems is not just genetics. Its stuff like hypothyroidism too for example. Sure you can be hypo and fit but it requires a lot more work. What you've described is the insulin hypothesis! Early life hyperinsulima creates a huge permanent tendency to gain weight, and makes it virtually impossible to permanently lose. let me ask you, have you tried losing weight? how'd it go? and when you relapsed, did all the pounds come back super fast?
i'd suggest that, you should, if you haven't at least, try a ketogenic diet + leptin supplementation. i'd be very surprised if you weren't blown away by how easy it was to lose everything.
