Different sports, combat styles, physical activities, etc require their own unique muscle build to be efficiently executed.

There seems to be a whole lot of offscreen time where he's just resting and shit and he seems fine. He's not working, he's pretty much just either dying horrible or heading somewhere to die horribly but most of the time he's either with Rem or similar adorable beauties before dying and watching them die.
He's not exactly fighting a war, can't really get that great at killing, and the thing that seems to bother him the MOST was the fight with emilia as he flashbacked to that this episode before going for the footlick.
He's having a much easier time then I think we would believe, even though he seems extremely emotionally affected in many scenes where dark shit is going on he recovers quickly enough.
Given the spoilers for his future performance, I think its fair to say that he's used to it. He's gone beyond suffering, its all just scar tissue at this point.
