At some point Tsukishima stabbed Yhwach and it fucks with his memory. He returns only to become victim to Hirako's bankai, which reverses the flow of time until he is a baby. Ichigo and Renji kill baby Yhwach. Also Unohana and Oetsu were the ones who lead the hunt on the quincy in the past.
