Nah most PvP non casuals would know about baiting and faking reactions, and not be stingy enough to not cast health identify spells.

With CBG in charge, they can teleport people anywhere they want. It seems they were distributed between the 1-3rd (Parupatra, Heavy Masher) 6th Forest (Hamsuke and Lizardmen) and 6th Colosseum (Foressight), but you could easily just distribute them throughout the hidden kill rooms like the one where the Greater Graveyard was waiting.
The only guaranteed paths are the floor to floor exitss, and all the rest will probably randomly teleport invaders around to disorient them.
And given the horribly unfair fates of what happened on the 8th floor, I suspect that nobody will ever be able to organize a big raid on Nazarick again.

Maybe that's how the dungeon bosses work, so a level 90 boss is more dangerous than a level 95 random monster.
In Guild Wars 2, at least, boss monsters are Legendary-class, while average monsters are only Elite-class, and the former are nastier than the latter despite being about the same level.
