I usually draw for 3-4 hours on a medium intuos with little to none pain, you might have posture problems or you're gripping the pen too hard. Take small breaks and do stretches.

I can tell you as someone who's had a pen & touch small AND a cintiq that it doesn't change much (if anything, it gets worse with a cintiq, because you use much more of your body with it since generally you use a cintiq in a vertical configuration).
