You're right. Guts in Berserk, for example, isn't even 16 when he meets Griffith and goes through the first half of the Golden Age arc, and still hasn't scratched the late 20s, and he's certainly no boilerplate fantasy LN hero.
The inverse is also true. It's not easy, but it's not difficult to name MCs in their 30s that are pretty much only distinguishable from your generic 16-year old MC by the fact that they can grow a light stubble and drive a car, but otherwise fall into the same old bland characterization.

I only used NG+ and post-quest as terms because it's much shorter than saying "a story where the MC is already a hero and circumstances force him to don the mantle again either to reach a better resolution or deal with some new threat".
