I believe some anon tried to do one earlier this year but failed to deliver the results. Most questions aren't mandatory so you can skip all the boring shit at the start if you only want to vote for you favorite anime, but since statistics give me an erection, and I plan to do a little correlation study at the end, it's encouraged to fill most of it.
Since some of the answers are directly typed in, if possible:
For anime titles, copy the romanization used by anidb.net, or myanimelist.net as a last resort.
For manga use mangaupdates.com.
For movies use imdb.com.
For visual novels use vndb.org.
In the case of doujin music, use vgmdb.net

Prelude tries to tie foundation with the I, Robot series by pulling out a deus ex machina of "the robots did it". The actual book is pretty dull, it's basically an 'on-the-run' style adventure through pre-collapse Trantor and is 90% heavyhanded worldbuilding exposition and some not-so-subtle social commentary on government and educational institutions.
It was so radically different from the rest of the foundation books it's hard to call it part of the same series.
