You're ranked compared to all the players of that current match. If you're getting gold medals (i.e. being the best on your team) then you're more likely to be more highly ranked. The game will notices that you're excelling compared to your teams and as a result place you higher for your real matches.
Competitive mode is a blessing. I've won all my placement matches so far and have been matched up with competent teammates. I've been playing Reaper the whole time and he is fucking INSANE when you're able to depend on teammates to take advantage of your flank attacks and create reliable defensive lines you can work for sneak attacks.
