I just deal with it by putting as much variation as I can. Change voice pitch, avoid duplicate hair styles and color combinations that look too similar with certain personalities.
At least the yandere and onee-chan personalities have added more to the variety. There are so many combinations you can do with just one personality alone if you can get creative.

Unreal is more suited to medium and AAA studios making PC, PS4, and XB1 games. It's capable of significantly better graphics, better animation, but runs like shit on mobile. The engine core is based on C++ (performance). A lot of known Jap studios have abandoned their in-house engines and moved to Unreal - Dragon Quest XI, Street Fighter V, Tekken 6, Ace Combat 6, and the FF7 remake are all jap games that use Unreal. Unreal also makes its own games using its own engine (Unreal Tournament, Gears of War)
Unity is more suited to indies and small devs making casual and 2D games and runs much better on iOS and Android. 3D open world games is not what the engine was built for, but it has gotten better at it. The engine core is based on C# (ease of use, modularity). The important part is that years ago Unity showed up as a cheap engine almost anyone could use while Unreal cost millions of dollars to license. That has changed now. It's easier to get up and running in Unity, and more forgiving of bad programmers.
This doesn't mean that a single person can't make a game using Unreal or that AAA studios don't use Unity.

This might actually get tricky for eroge devs. Unity just recently eliminated its perpetual licenses and went subscription-only. Unreal stopped licensing for millions, but started demanding 5% royalties on income above a certain amount. With the subscription model, they might start caring how their engine is used, and by whom. Same with Unreal, who takes royalties.

Unity switching to a subscription format is most likely due to some unity games becoming unbelievable surprise hits and they would / will make a lot more money off that than they would off a single license.
I don't think eroge devs will run into problems here unless unity becomes compelled by law to list everything made with the engine in some public list and even then, if they make that list difficult to decipher, they might still not care. Then who is going to care what an engine is used for? In the end, engines are base tools and people won't care about that in the same way they would care about IPs or characters. Nobody is not going to buy an unreal or unity based AAA game they want because some japanese devs made a porn game on the same engine.

There is a Photo Shoot mode that has limited application due to only the absolute base game motions being available (far from all sex motions) and it only lets you rotate the maids and move them around (and re-size) but it does not let you adjust their arms and legs and what not.
Multiple Maids is a mod plugin that lets you do a lot more than Photo Shoot mode but needs a lot of practice to make good looking things with.
