It can be not-failed, but you'll lose Dijkstra's support in Kaer Mohren.
The only way
to get 100% is to start Gangs of Novigrad, don't meet with the dwarves, do everything stealthy, and after that meet with Cleaver. You'll get two red crosses, but the quest will be marked as finished.
Then you just have to let Menge torture Triss and ask him about the treasure.
And you get 100% game, with Dijkstra's support, and Triss getting what she deserves.

There's a lot of hidden shit in the game.
For example, there's an unmarked cave in Skellige, where you can find a poet who's in love with a grave hag and there's a very short quest there. Also, on the eastern shore of Ard Skellig, near the ruined forest, there's also a quest.
Some of the quests aren't even in the prima guide.
In Hearts of Stone, when you go to the Herbalist hut when starting the quest with the heist, you can encounter a bloody boar, it leads to a small quest without a journal entry, but it doesn't really conclude anything. If you just fast travel to the Herbalist Hut, you'll never get the encounter.

I have a morning ritual that I need to share. I call it "the witcher". First I sit down in the shower in the classic "Geralt meditating before a contract" pose. With my eyes closed I sit there for a minute, visualizing either Geralt or the big guy from the second game (not the chick in the third one because that one sucked) and I start to hum the dice poker theme. That's how Poland was fucked. We lost 30% of population during war (3,7 mln of Polish Jews and 2,3 mln of Polish catholics), most of our country was turned into a fucking black hole and steaming pile of rubble. 45 years of communism pauperized us so much that in 1990 we were poorer than Bulgaria.

There is a tiny island with nothing on it in Skellige. Hidden in bushes, you find a dead boy and next to it a note in gibberish.
This leads to a quest which takes you to an unmarked cave near Novigrad after searching for some shit.
Also there's a cave in Skellige which is entirely dark but has an overpowered venomous arachas and a whole bunch of other critters. I don't really know what's in there because they killed me within seconds, but the cave seemed big.

So I downloaded an old reshade preset and overwrote dxgi.dll without thinking about it and the game crashes on startup now. Looked for a download of it but can't find anything and silly me forgot to make a backup of it.

Right now I am doing a Death March re-playthrough and my main advice would be to only invest points into things you want to use. Read their effects and see how much they will assist you depending on your build. Dabbling is fine but try not to put too many points into random perks. Also, try to match 3 perks of a color with a mutagen, the boost is worth it.

At a couple points you'll have opportunities to buy potions that will reset your skills. You can dabble if you want to, but if you're going Death March then you'll have a rough time if you don't spec right.
If it's the second hardest difficulty, feel free to experiment a little and respec when you know what you're doing.
