I'm doing my first walkthrough now, right now I'm at the cainhurst castle and I figured the game will be over soon. I've mostly gone through the game blind but when I checked if I have fought all the bosses, I noticed that I've skipped about every NPC questline there is.
I'll by the DLC next month, should I play it in ng+ or just start a new game? My character build is most likely horrible, a properly built character would be nice too.

Not even close. Bloodborne is here to stay. Its tight as fuck gameplay made sure of that. The ones who suggest that they are leaving will come crawling back to us once they remember how clunky it feels comparatively. I love DS as much as the next guy but Bloodborne spoiled me.

Soul level 64, just unlocked the shortcut to the cainhurst castle area. I've been using the saw cleaver for the whole playthrough, tried Ludvig's holy sword and kirkhammer but didn't really like them, although the former is my backup weapon. I put some echoes to arcane because I wanted to use the beast roar, but honestly I haven't had a clue about how to build a character at all.

You're fine. Vit needs to be at 40 minimum but 50 is best. I think 20 End is enough because you can find runes that buff your stamina. You can go higher if you want to. Keep Str at 25 if you like faster weapons because that's where it soft caps. 16 Arc is the sweet spot because this would allow you to utilize many hunter tools. Remember, you can block any projectile with Beast Roar. Since you like the cleaver I recommend the Beasthunter Saif which is found in the first area of the DLC I think. When you find a weapon you love, check its attribute bonuses to know about which stat you need to invest points in for your next build.
