The thing with the spear is, despite the sprite's size, it's hitbox is only half of that. It starts at the silver part in the middle, so it's pretty much Mom's Knife, except you can't throw it.
In The Guardian, even though it's been reskinned for reasons, the hitbox is the same, and it's harder to see where that hitbox is. Again, because reasons.

i usually play greed mode with isaac or azazel (just to clear the rooms faster) and after i got a devil room on sheol with three items (none of which were mega blast), i used d6 once, still no mega blast, i had the battery which gave me an extra charge of it and once again no mega blast
i had seen maybe 12 devil room items in that one run and didnt get it

Two ways to win. Either you need to be able to bum rush through the game, meaning you want a Floor 1 Dr. Fetus, Ipecac, or Pyro (just skip majority of rooms by blasting the doors), or a Floor 1 We Need to Dig Deeper!
The other way is extremely careful defensive play. Floor 1 Holy Mantle can allow you to completely bypass the 16 minute time limit. After your time is up, you recieve damage every 10 seconds, but that damage can be absorbed by the Mantle. So long as you can clear a room and wait for the mantle to take the hit before moving on to the next room, you can keep going forever past the time limit.
Or just having a really OP build to where you can tell the game to fuck off. Tammybrim or something.

Alot of the challenges have bogus, or outright harmful, rewards.
I'm personally saving as many meh challenges til after I finish all my post it notes. There's alot of advantage to a small rune pool with only a few select runes in it, as well as not unlocking useless pills or certain cards.
I'd say the challenges worth doing are 2, 6, 8, 9, 10, 14, 15, 17, 18, 19, 21, 23, 29, and 30.
Outright avoiding 1 (Hagalaz seems the most common Rune, so keeping it out the pool helps), 7 (card pool, very rarely useful), 11, 24, and 25. All others are either not directly useful, but not exactly harmful either.
Alright, I have super bandage girl/ super meat boy, all challenges, all characters unlocks on hard mode and I have 3 achievements left, other then finding every item in the game, wtf am I missing?

Possible that your missing some of the do X thing achievements? How many times have you died? Got all the _ Boy achievements? (I assume that's automatic for doing the Lost though). Fully upgraded your shop donation machine? Got all alt-bosses unlocked etc? (I'm nearly done with the game and only just now unlocked Triachnid for example).
It'd be easier if you just went into Steam, Library, View Achievements, bottom of the page is the ones still locked.

No. Taking a devil deal locks you out of angel rooms appearing, and having had a devil room on a previous floor lowers your chance for one to open up on the current floor (even lower when you had one on two of the previous floors). That's all it does.

Getting a devil room/angel room automatically sets the probability on the floor after that at 1/3 and increases by 1/3 every time you don't get either. Getting a devil deal precludes angel room from spawning. Joker cards devil rooms do not count, you have to spawn them after the bossfight. After an xl floor you have 2/3 probability of getting another angel/devil room. That's it I guess, except sacrefice rooms and killing beggars

Basement 1 has 0% devil deal, basement xl has 100% as it is for basement 2

Floor 1 has 0% devil/angel room.
Floor 2 has 100% devil room.
Floor 1/2 XL obeys the same rules.
Getting a devil or angel room reduces your chances for the next floor, but that penalty decreases the lower you go. The baseline is thirty-something percent chance to get a room which doubles each floor you don't get one.
If you use missing hud you can watch the percentages yourself.

Curse of the Tower is fine. You don't want it on Basement 2 since it effectively locks you out of blood banks and demon beggars but on the later floors it's free damage since Polaroid invincibility lasts longer than it takes for all the bombs to spawn and detonate.

The keeper is pretty shit, but once you get all his unlocks he's not nearly as bad. He gets a ton of money from the wooden nickle and the shop key is nice. Plus if you find swallowed penny you just win. His only huge downside is his fucking awful tears down from triple shot and that he can't take devil deals.
