Arturia had already been summoned. As far as summoning timelines go she was the first to be summoned. The EInzberns have always summoned really early. It's why Diarmuid's a Lancer. I'd bet money that Kayneth was trying to summon him as a Saber but got keked by Kiritsugu into Lancer.

His disguise ability only hides a few factors, which is why its not smart to transform yourself into a normal human when you've still got readable servant levels of mana.
Its one of the reasons they had Berserker turn into Alexander rather than just Maiya or something to do it more covert. Also because he can't really replicate any of the items that person had, which at times can get him busted pretty fast as "Not that person". On top of disguising himself as Rider to instigate war against Rider.
All disguise abilities to date can't really disguise everything about you, they always leave one gap that can be seen through.(Except Li Shuwen). Maybe one makes it so you can't be seen, but you can still be detected by smell, or maybe one makes it so you can't be seen but you can be detected by sound. Also there are types that lose effectiveness when you attack. Even now there's one where it doesn't really hide anything about you, but instead just makes you be seen as a non-enemy.
Its maybe said Robin Hood's Faceless King is about the most broken hiding ability in the series right now, and it disguises everything except you can still faintly feel his mana in the air when he's nearby. You can't smell, hear, or see him, but if you can sense mana very well you might still find him.
