I once got so mad at Dishwasher: Dead Samurai that, without thinking, put my controller in my mouth and bit down on it.
5 hours at Sen's Fortress, literally screening at my screen since I had resorted to just running as far into the level as a could making a mad dash for the bonfire.
Runner up probably grievers in csgo. I've proably unfairly chewed a few kids out, but such is life.

I think it was the last level of Catherine, I was having such a hard time with it. I think I spent a good 1 hour stuck on the stupid level, I wasn't very good at the game to begin with so that didn't help.
After 40 minutes went by, dying over and over again, each death made me more and more angry, swinging my arms around like a wild animal, swearing at the top of my lungs, no game had ever made me this angry.
Eventually it got to a point where I was no longer "angry" I just sat their, playing the game, dying over and over again, the rage had just bottled up inside and I was staring at the screen with a blank expression.
A friend of mine was watching me play and told me later that he was genuinely scared to be in the same room as me and kept quiet the entire time.
I beat it eventually by sheer luck and felt absolutely nothing, I wasn't happy it was over, that I finally beat it, I just kept on playing like nothing happened.
Ever since, a game has never made me angry again, not once. That last level broke me and now I no longer get angry at games, ever, I even make fun of other people when they say they break their shit because a game made them angry.

Anger has blanked out every memory of the game except that I was trying to capture a zapdos and something fucked up and I lost my chance and I chucked my controller(using super gameboy) so hard on the floor it literally exploded.
Dark Souls, they aren't the hardest games when you get to know them, and the bosses are usually pretty fun and fair
When a regular mob combos you to death because you didn't look around a corner at the right time or something is so damn frustrating, it's the only time when I legitimately have to take a break from a game and do something else for awhile

hell if anything I am the cause of peoples anger because I like to play 'competitive' games like Dota but I have a carefree, don't give a shit whether I win or lose attitude and that attutude just does not mix well with Those type of communities

One time playing Armored Core: Project Phantasma I was having a real tough time with this one mission, when I suddenly realized the enemy AC had run out of ammo and was defenseless. Then a bomber plane that was supposed to be allied to me drop a bomb on my head and killed me. I grabbed the controller by the cord and slammed it into the ground repeatedly until it broke.
