That's a huge topic to spoon feed you, but here's a quick rundown.
First off, the insurgents will almost by definition lose most of the fights and take the most casualties.

But the main reason why modern nations fail to hold these shithole countries boils down to basically one factor.
Their populace doesn't want them to win.

Basically what they would do is round up all the civilians and put them in camps.

While these camps had bad conditions at first, some of them ended up with lower mortality rates than London.
If you tried this nowadays, you'd basically be compared to Hitler, and called a war criminal, ect.
Public opinion also forces the modern country out, even after they've won.
We saw this in Vietnam, and we're seeing it again in Iraq.
At the end of the Vietnam war, Nixon bombed Hanoi until they finally agreed to negotiate for peace.
We signed the Paris Peace Accords which established peace in Vietnam, just like how it was before the North invaded,

Actually, that was standard operating procedure for pretty much EVERYONE in those time periods. If you didn't surrender your city, you were going to get looted and pillaged, and even if you did there was no insurance the troops wouldn't run wild through the streets.
