A creepy silhouette will do the job. Fear is sometimes built on hype and if you do your best at verbally describing the monster without showing it, the greater it climbs.
If you didn't already know, horror is mostly composed of acting. Acting afraid despite staring at nothing but CGI or animatronic, etc. They have to put it in their heads a belief to experience fear. And that belief comes straight from the writing.

None of you would calling it "one of the greatest psychological horror movies ever made" if the creators of Silent Hill never mentioned it. If they had said they were inspired by Flatliners you'd all be running around call that "a top 10 horror film".
And the funny thing is all they did was rip off some of the superficial imagery from Jacob's Ladder. The game and the film have very little of substance in common.

It's called Back in 1995, it's a indie PC game from Japan developed as a homage to old school survival horror. It's not really that deep though, it's kind of like a tech demo.

Most horror themed games aren't scary after a time, and many times, not scary in the first place despite containing all the spooky elements you'd expect from a movie.
The scariness and tension in horror games are created by the mechanics more than they are the other factors. The game needs to make the player feel disempowered, as if they're constantly balancing on a knife's edge for a game to be sufficiently "scary."
