I'd love a Fallout game set in Japan. Since aesthetic style of Fallout in US is sort of pseudo-future and Mad Max, maybe Japan could have gone into different direction, with big cities having technology, while most of country is made of villages like what you'd see in 1800s.
The mountains and villages between them would lend themselves wonderfully to exploration style gameplay, and with the country being surrounded by irradiated sea pretty much seperated from rest of the world, you could also do some interesting pseudo-fantastical elements.
Maybe some people mutated to look like folk-lore creatures like kappa or tengu, and settlements would have to fear attacks of giant monsters that raise from sea or mountains?
American Fallout is all about high-tech, so it'd be interesting to adventure in lower one, with developing railways, etc. Lack of actual nuclear energy would also circumvent the stigma of presenting that in Japanese games.
But sadly that won't ever happen properly. Whenever developer tries to make a game about culture they're not familiar with, it just feels wrong, so you couldn't really have western developer handle it.
Similarly, Japan doesn't really have developers experienced in open-world RPGs, like what you'd expect of Fallout games. It's a shame, really.
