it was when you meet that philosopher super mutant in a random encounter in Fallout 3. when you tell him to give up his things and he talks about how he wishes he could give you the moon instead. that was a long time ago though, i played it again recently and it didn't make me sad.
In the Witcher 3 when you get to the isle of mists and Geralt thinks Ciri is dead, that was pretty great. His reaction is realistic, how he recoils and then just rocks her back and forth, I teared up.
The ending I got was Ciri being Empress and that was pretty good as well, hit all the right emotional notes.
Witcher 3 is one of my top 5 games of all time I think.
