The game is a rollercoaster of difficulty. The biggest problem is that storyline quests give you a retarded amount of experience so it constantly makes you outleveled for the game. But then you might run into a ridiculously hard fight.
They added a level scaling option at least, now, where it will up all enemies to be your level. Haven't used it much, I probably will after I beat Blood and Wine and want to go around all the regions finishing all the sidequests.

It's funny because we used to hate Elder Scrolls over that. But I think it's because it went both ways, where enemies would be piss easy even in areas that should be hard.
