As a classic Fallout fan it is important for me to note
that Gamebryo is broken shit and nuFallout is trash
however I am prepared to admit that at least Fallout 3 and New Vegas had a shitload of new content added in their respective DLCs. If you wait a year or two after release of a new Fallout game and grab the inevitable GOTY edition on sale for $20 you're in for quite a few hours of gameplay.

Well it's sure as hell not Left Behind, and I like Left Behind.
For me, it's Dead Money, followed closely by The Shivering Isles. The Lost Chapters for Fable would be my answer if it had been DLC and not a re-release of the game as a complete edition. Broken Steel is close because it allowed me to play past the ending of 3, but the actual content that it included was mediocre, and un-fucking the ending was its only saving grace besides maybe Hellfire armor.
Artorias of the Abyss could be my answer if you were to only think of the bosses; the actual "new" area it added, and the vanilla enemies therein, were pretty shitty IMO. I'm just now really getting into Bloodborne, so I haven't had the pleasure of playing The Old Hunters.

Really not Obsidian's fault. They had to use what they were given to work with, and they did so to the fullest extent that they could. I'm actually pretty amazed that such a richly featured RPG came out of NV's development. It's kind of a miracle, all things considered.

Time and money, anon. The Legion going unfinished is the biggest tragedy of that game next to it being forced onto Bethesda's shitpile of an engine.
From what's been said since the game released, it would seem that Obsidian had to spend a lot of time fighting with Gamebryo and getting shit to work. Time that could have been spent doing literally anything else. Doing simple, normal shit like getting the lever action weapons to reload properly was a bullshit puzzle in Gamebryo.
So not only did they have to deal with Bethesda's patchwork way of developing games, but they also had to plan a complete RPG with every line of dialog, every skill/stat/faction check in said dialog, every quest, every quest resolution path, all this new goddamned lore. It was beyond ambitious of them to shoot for such a grand, balls-to-the-wall RPG while trying to overcome developing with such shit tools. Couple that with the power outage that killed four months of progress, Bethesda tying their payment to fucking metacritic, and the short development timeframe, and you get an unfinished game.
And because Bethesda are tight fisted jew bastards, we'll never see New Vegas finished, or get a followup game from Obsidian. Beth are salty and it shows, they'll never let anyone touch the mainline again. Maybe they'll whore out the IP to an MMO dev or some shit for subscription cash like they did their baby, The Elder Scrolls.

No, they are. It shows. They took 4 as far away from New Vegas as they possibly could, even omitting gameplay features from 3 that were improved in New Vegas. No skill or stat checks, ultra simplififed leveling, guns are all cartoony nonsense, the story is all about family problems, pre-set backstory, voiced dialog, ect. It's all just a big "fuck you" to anyone who might actually like RPGs. I mean, shit, Pete Hines likes to get on Twitter and shit on the lore.
Obsidian basically took their permission and budget to make a spinoff of Fallout 3 and made the actual, real Fallout 3. In terms of development, Beth got showed up hard. They can look at the metacritic and sales and call themselves as superior as they like, but the fact of the matter is that Obsidian absolutely kicked their asses at making an RPG, so they decided, out of spite, to make Fallout 4 a CPG-FPS instead of an RPG.

I have no doubt that work was being done on the game as soon as 3 was done, but not the bulk of it. Considering that 4 has very little actually shit to do in it, an empty as fuck map, few unique assets (it's all just the same shit copy/pasted everywhere), and the most lazy main quest to date (5 ending slides, are you fucking kidding me?), they probably did most of it in a few months. Beth taking a bunch of reference photos of Boston doesn't mean that they had their nose to the grindstone all that time. 4 is not an RPG, and manages to have less to do than 3.
The actual content of 4 is pathetic. I shouldn't have to hunt for an actual side quest. "Radiant" repeating quests and trying to push the minecraft mode is the laziest, shittiest thing they've done yet. I can't take two steps in New Vegas without landing in several side quests, and 3 had secret shit to find out the ass. 4 seems like they waited until they needed some cash and threw it together with their previously collected Boston stuff in about a year.
Ironically, one of the worst things about it, the voiced dialog, likely had the most effort put into it.
