Depends on your definition o 'combat difficulty'.
There are mods which do noticeably improve the AI, some examples of such:
Enemies actually react to sneak attack criticals on their friends and seek out attackers
Ranged enemies use cover more
They will actively seek out stronger weapons if there are more in the area
And you can also improve their aim and give all human NPCs stimpaks, which they will use when low on health.
