Assassin's Creed Unity
Got completely enthralled by the series as a whole, was monumentally excited for Unity judging by how it looked and played, and having a PC that could run it on high settings.
Got it on release, played for 5 minutes, computer shuts the fuck off completely. Continued to do this until I replaced the thermal paste a few days later. Even then, bugs and glitches bog me down, multiplayer connectivity is frog ass, best weapons and gear are locked behind dumbass community challenges that aren't even accessible without joining a klan, and some gear even needs that stupid phone app to acquire.
It left a terrible taste in my mouth, killed my inner fanboy forever, I haven't gotten/played syndicate either. Other than that, never been seriously dissapointed by a game.

Some people just haven't had their spirits broken yet.
Skyward Sword was the nail in the coffin for me. ME3 and Thief helped. I haven't been able to look at a new game without searching for inherent flaws since, it's very disappointing.
Stellaris. Great early game but a combination of the retarded sector AI, UI quirks, and a completely barren midgame means i haven't touched it since that first week.

It's okay, I'm the same.
Nothing traumatic, but Dark Souls 3 was a bit of a letdown for me.
Worst PvP, worst covenants (mound makers are a cool idea, but people are fucking boring and rarely turn-heel), areas are mostly thematic or literal retreads and it has embarrassing texture quality, even at max.
That aside, it has my favorite bosses in the entire series
