The covenant you join changes the icon of the "etsus" healing item. One is rolls, another is flasks, and so on. It's just a cosmetic difference.

Ten to fifteen hours just doing the required bosses to advance. Longer for doing optional areas and bosses, crafting, or leveling for special weapons.
There's NG+, and the skill tree is also fairly diverse giving some incentive to try out different play styles with different characters.
There are covenants to discover and some side quests to do. You'll probably miss some of those on your first play through.
If you enjoy Metroidvania then you'll like this. But don't get it just because you like Souls games. This wont scratch that itch because even with all the stuff they rip off from Souls it's still not a "2D souls." It's Castlevania with Souls shit thrown in.

Not entirely cosmetic. The rolls and red potions from two of the beginner covenant choices are just basic heals but Devara's item heals you and increases your defense for a short time after drinking it. The gold covenant's wine will heal you but will poison you if you drink two in a short time. The betrayer covenant's healing item increases a hidden corruption stat and past a certain point every other covenant's NPCs become automatically hostile.

Yes, to an extent. When you start the game you are given the choice of three covenants to join. When you join a covenant you are given an item of that covenant, and each time you find an empty sanctuary you use that item to claim that sanctuary for your covenant. Once a sanctuary is yours, you can use statues to summon NPCs to that sanctuary, such as vendors or fast travel guides. These NPCs, depending on their type, also give a particular bonus to the area that the sanctuary is within. The magic seller gives a magic bonus to the area, and so on.
If you find a sanctuary already claimed by another covenant, you can heal and level up but you can't place NPCs or fast travel to it or from it. There are items you can get in the game that allow you to retake an already claimed sanctuary. To change covenants, you have to find a sanctuary claimed by the covenant you want to switch to and then take an oath to it.
