"gg" is a customary sign that as far as you are concerned, you believe the outcome of the game was fair, or at least according to the game's rulesets and that you accept the outcome.
When you don't "gg", you basically imply that you are not willing to accept the outcome, either because you are frustrated with something, or you think the game did not occure as it should have, such as the opponents exploiting, cheating, displaying poor sportsmanship.
Everything else is basically a meaningless taunt that you should never pay any heed too.

First of all, that is not true. There are some who do that, not an insignificant amount, but not most. Not to mention: what the fuck do you care? The gg is there to communicate your own issues or lack of issues with your view of the match. You should wonder what went wrong when you don't get it (see if you did not do anything to make the other side think it was a display of poor sportsmanship): you yourself should use it to communicate an issue with the other side's play.
When you are getting it, you really don't have to fucking care whenever it's serious or ironic. You either accept the outcome as fair and in line with how the game should be played: if it is, you type it (it makes more sense to write it first than the other team when you lose - fairly - to indicate you aren't a sore loser, but it does not matter that much), if it's not, you live without saying anything - and you either get it from the other team (in which case you wonder if you haven't done something wrong), or you get it and you don't have to wander about shit.
It's also worth noting that in some games, "gg" is flat out used as an admission of defeat when the defeat is not obvious from the communication of the game (RTS games are a good example) - again in line with the "I accept the outcome and consider it fair".
That is all you have to worry about. Anything else does not concern you what so ever.
It really is a simple guideline:
Get "gg": don't think about it.
Don't get "gg": ask yourself if you haven't done anything wrong, then move on.
Found the result of the game according to customs and rules? type "gg".
Found the result clearly unfair or in breach of the rules? don't type "gg".

gg is the equivalent of a handshake at the end of a chess match, or the bow at the end of a duel in Asian martial arts.
It exists to show that you harbor no resentment against the opponent and that you respect the outcome. It's a good way to communicate whenever there was something wrong with what the other side did or not.
