If Fates had no objectionable content aside from the minigame, then it would've made it out the door with a T no problem. But even the censored version definitely earns its T rating, at least as far as the ESRB goes. You can still strip characters down to their skivvies, there's still plenty of suggestive dialogue, and there's still things like Ryoma's corpse smoking after he committed sudoku with an electrified sword. The minigame would've just been the straw that broke the camel's back, so to speak, pushing it into M territory. You and I may not have cared if it got an M, but Nintendo evidently did, hence the decision to nip it in the bud beforehand (for better or worse). Who knows? The ESRB is pretty inconsistent at the best of times, but Nintendo evidently felt that the game had a realistic chance of getting an M, hence the self-censorship.

What you describe simply doesn't exist. I work for a video game publisher (we're not huge in the West, 1~2 big release per year top), still the way the rating submission work isn't like that.
There's no such thing as "thought it would", ratings are agreed upon both parties (works exactly the same for ESRB / PEGI or USK).
1. We review our game internally first and ask DEVs if they believe that something which could result in getting a higher rating is present in the game.
2. We submit a version of our game that is representative of the final product, the rating we want and we highlight any potential issues (i.e. : during quest X, NPC Y gets killed and it's a bloody scene etc.)
3a. We await for the first review. If the rating we asked is approved then good and we move on.
3b If they have issue with certain things then we start discussing about changes we should to just to get in line with the rating we want while keeping changes to a minimum (not because we care about the original work or the author (nobody gives a fuck), but because we don't want to overburnden our DEVs and waste time). Changes are never ever forced upon by the rating agency everything is discussed. We have an extremely friendly relationship with people reviewing our games and we're small. So a publisher like Ninty must have it real easy.
As a publisher we are way more concerned by media and journalists and the "public opinion". We rated many of our product M while they could easily have been a Teen. Fact is we even have games with a M rating in the US and a USK12 in Germany (and Germany is extremely conservative about ratings), but because German media don't cause us issues on specifics things, we can just get by with lower ratings. Likewise for the rest of Europe and PEGI.
PR is our biggest issue and like in every company our PR director is afraid of any potential scandal / negative article toward our products.

Not really, that phase is completely internal and has no impact what so ever on rating. Even if you believe that X or Y "may" get you a higher rating, it doesn't matter you still submit your game asking for the rating you want and you wait for the review.
We have to ask that question to DEVs simply because as a publishing team we aren't aware of 100% of the content present in the game. ESRB/PEGI/USK won't also play your product in its entirety, so it's our duty to provide them with a list of things we believe could cause issues.
Rating submissions are also done at a late stage of product development and the last thing you want is having to rush to make changes (if necessary), so it's better to know in advance what could potentially cause issues.
Again the risk is always: we get a rating lower than what we should have by not giving all details to the reviewing agency, some press person makes an article about, it gets picked up, we get in trouble afterwards... I doubt people purchased it because it was censored. Rather, I believe it simply sold really well because Fire Emblem games increased in popularity after Fire Emblem Awakening and it recieved a lot of attention, such as from Smash Bros.
