Why do you look down on Katawa Shoujo?
Visual novels are very niche (outside of Japan at least, probably there too). This means that the community of people who like them is very elitist. Up comes a VN with a unique twist which is notably written in English originally and not Japanese. This VN becomes quite popular for these reasons and it introduces many to VNs in general. Suddenly this elitist community is flooded with people who have only read a single VN and declare it to be amazing. You see where I'm going with this?

Nothing immediately comes to mind, but the general idea of girls with some problems, psychological and/or physical, is a staple. KS just takes it further.

If you want to get technical it wasn't entirely original either. The artist Raita (Mahou Shoujo and Valkyria Chronicles) drew the concept for the girls as a joke in one of the porn doujins. Since the word "Katawa" is actually a derogatory term he denies involvemen too.

I wouldn't say the girls' disabilities are fetish fuel. I think that's something that KS did well. It's explicitly and repeatedly made clear that despite their physical abnormalities the students at Yamaku are normal people. Even the sex scenes which one would imagine to be the most egregious instance of disabilities being used as fetish fuel aren't focused on them at all. For example in Emi's scenes her lacking legs doesn't come up at all. She even is on top of you for one of them despite the inherent difficulties.
