That was actually half the point of her dressing like that. Even naked she'd be tough enough to take on one of those military mechs, so she dressed like a stripper to distract people.
It was supposed to show how she had radically depersonalized herself and saw even her own body as little more than means to an end. Take note of how she treated the Tachikoma like expendable objects as well whereas Batou, who was clinging to his humanity much more so than the Major, treated them somewhere between children and pets.

That's a purely unit and environment based thing. The OTV's and IOTV's we had on our deployment had the neck guards. Not everyone wears the plate carriers. In Afghanistan you aren't as exposed to anti-personnel IED's as in Iraq, small arms fire is the bigger threat, and the terrain is more exhausting, so people wear plate carriers there more often, while in Iraq, our armor was more based around defeating shrapnel(And thank fuck too, shit saved my life, personally).
It is pretty heavy and uncomfortable as shit, though, which is why they've been trying to find a middleground between the two types of equipment over the years. Modern soldiers carry way more weight than, say, people in Vietnam or earlier.
