Adds a lot to the experience IMO. Not saying the game's good, just that it feels a lot better when you have to consider food, drink and sleep, the fatigue, diseases you get from bad food and certain enemies, the stat effects all these things have... plus, no saves unless you sleep, no console makes the whole thing really immersive. Instead of quicksaving and rushing into a room, I had to really consider "fuck, do I really want to do this brotherhood quest now? a Synth could kill me any minute."
It also makes the settlement building worth it as it adds bottlable water mechanic, and you can't fast travel so you're actually incentivised to use a settlement in a region as a base while you're doing some looting/missions in that area. It felt good doing some quests around goodneighbour and actually walking back to the hotel for some sleep(saving plus XP bonus) before going out for the next day, buying food and supplies from the shops that you actually need.

Played a pirated version for a weekend. There were some fun parts, mostly in settlement building and weapon customizing, and a few mods to clean up the mess and performance, but I just couldn't get past the feeling that the game felt half-finished.
