Should I get Dark Souls 3 on PS4 or PC?
My PC would only run it about as well as PS4.
I don't plan to upgrade for years.
I use a dualshock 4 for my PC games.
I already have a PS+ subscription for the next year.

PS4, definitely. You'd already be playing an equivalent version to the PS4 on PC (Dualshock 4, Medium settings) except you'd be getting fucked by hackers out the ass.
PS4. If your pc is only capable of running it at ps4 levels then there's no real point. Besides the souls community sticks around for years on Sony consoles. I still get regular invasions in DeS after close to 7 years.

I'd say PS4 then, all other things being equal. At least you won't have to deal with hackers. Maybe you could have faster loading times on PC, assuming that you have a SSD. I built myself a new PC solely for DaSIII.

Well being that that's literally the only argument console kids have against PC users, this really shouldn't come as much surprise to anyone.
You're more likely to have performance issues on a not-so-great computer than on the PS4 (even though you'll probably have performance issues on the PS4 as well). So PS4 I think.

DeS was only "best" in the sense that it was the first game in the series, and while the new games are certainly recycling the core gameplay they've also improved it with virtually every installment. Going back to DeS feels quite dreary after playing the newer games.
The only objective strength DeS has over the other games in the series is core online mechanics being accessible more early, plus they're simple and something that was tied perfectly into to the single player experience due to the resurrection items being rare and having no bullshit grindy covenants getting in the way. Then again DeS also had plenty of unbalanced and broken shit (both PvE and PvP wise) so it's hard to call it the best online experience either.
I haven't played Bloodborne since I'm not buying a console for literally one game, but I figure it's the same thing with retards pretending that their platform of choice has the best exclusives.

The 750ti, which is noticeably slower than your gpu, runs the game at 30 and slightly more fps on average.

I think you should get it for pc. I don't own a ps4, but I don't think you can adjust graphic settings on console. Maybe on pc you can go all low and crank out 60 fps.
After playing blight town on ps3, I'm skeptical.
Also, you should upgrade. You can get a new GCard fairly cheap. I think there are some GCard bundles that include Dark Souls 3, so you might be able to squeeze more value out.
I've known quite a few people who have pcs that have rockets attached to roller skates, and it gives them a pretty good run for the price.
