With CS your character is always maintaining a cheek weld so even though 90% of your screen isnt ironsights its implied he is using the sights and trying to hit your point of aim
if you are standing still and fire one shot most guns will hit that point the first time
many factors can affect this like your movement speed, standing or the gun still recoiling from another shot
