It would need to be an online persistent world. Or at least.. persistent until new versions of the game are released.
It would be top down, of course. You wouldn't have direct control over your tribe, but you do have control over what they do as a whole.
For instance you draw a blueprint for a new type of shelter for the tribe to build. If they have the materials, skill and energy to build it. Then they will build it (with randomized changes depending on skill). If it doesn't work due to the environment or physics, then it doesn't work but they tried to build it anyway etc.
If you want them to build new tools or weapons, you again draw a blueprint for it.
If you want them to go and gather resources, or hunt etc, you can tell them to do that... Sometimes you will come across other player tribes, and if so they might fight. Or if the other player's tribe is vastly superior with higher living standards and more food, some of your tribesmen and women will join them etc.
It wouldn't need to be that complicated. It would just require huge server power and the right physics.

God, I love the secret world so much but its engine sucks and the character models are atrocious. I would love for a massively single player conversion of it, or some spinoff that takes full advantage of the setting. There's still so much that can be done with it. Complete with mic support and even online lobbies where people can make their own maps and quests through a map editor. All of the towns would be medieval times style and comfy as fuck. Basically just a somewhat open world fleshed out FF1 with online and good combat.
