First, they delayed the game last-minute in the West even though it was already finished (to improve their balance sheet for the next quarters, or something like that). It didn't release for over six months, which divided the playerbase between importers and people who waited. The people who waited then had to contend with people who'd already been playing online for half a year. Even Platinum had no idea the delay was coming; they found out about it the same day the rest of the world did - over Twitter.
There were numerous bugs (e.g. Baron's heavy Killer Weapon causing an instakill in medium-to-high-lag conditions) and infinite combos, but Sega refused to finance a patch. If anyone ever wanted to play, the threat of getting infinite comboed in any 1v1 situation surely put them off.
No advertising whatsoever - for a multiplayer-focused game with no split screen, it's vital to have as many people playing as possible. The less people are playing, the less other people will want to play as it becomes more difficult to find a match.
There's various other things like the day one DLC (including two GAME MODES), too many playlists making matchmaking unfocused and so on, but I only have room for so many characters in one post. It was a game with fantastic potential and unique gameplay, and it'll never come back.

First they switched from a PS, to an Xbox exclusive title. Then they put out some god awful DS game. Then they put out a god awful Wii exclusive (pic related).
How the fuck does this happen? All they needed to do was improve on Tenchu Z, and not completely abandon the platform their entire audience was built on.
How does this level of incompetence exist?
I just want classic (pre-Bamco) Namco to come back and give us more.
