The funny thing is that there are no laws regarding the ESRB. They're just guidelines that stores don't actually have to follow, unlike movie ratings where you have to be an adult or with an adult to purchase a ticket to an R rated movie.
I miss the days of no ESRB. It seems like they're getting stricter and stricter on their ratings every year, and devs are terrified to put out something with an AO rating.

They aren't. In the early 2000s, a game with a single pair of tits would be bordering on AO, but now they're fairly commonplace along with large amounts of gore and such.
Something like Hot Coffee probably wouldn't get anything more than a passing glance nowadays.
