The actual reason is pretty fucking stupid either way, though. It's clearly a situation where the story justified the design, rather than the design justifying the story. He wanted to have a female character in a bikini and wrote the story to facilitate that, and it was pretty fucking stupid, as opposed to just writing a good female character and giving her appropriate attire.
That's not saying putting a character in a bikini is always stupid, I'd just love if it were justified in a more human manner rather than "UH, NANOMACH- I MEAN UH PARASITES." Like, she's clearly a hotshot at what she does, so I think it would've made sense to give it more of a "it's hot in the middle east and I'm so good that I don't need my fatigues" kind of direction.

When you burned Quiet during the intro cutscene, she inhaled the flames and destroyed her lungs. So she was given emergency photosynthesis parasite transplants (Which was how The End lived, no supernatural stuff here) which also results in her breathing through her skin.

When I first got to her in the ruins, after that whole "You have to decide what to do with her, Boss, it's your call" radio conversation I just tossed a grenade at her.
I was actually kind of surprised that I could just redo the mission and recruit her. I still haven't used her on a mission, though, I always take my dog instead. Even without having anything spoilered I knew that she tried to kill me twice (at the hospital and then at the Abe Shifap ruins), the fact that she defended a helicopter that *she* was on doesn't really qualify as "she saved the Boss's life" to me.
As for her costume, I've got nothing against fan service. I have yet to deploy the Sneaking Suit (TB) or Jumpsuit (Eva) without them being unzipped. But the explanation for Quiet was really lame and she doesn't really have that great of a body. She's not bad, but she's no Shiranui Mai. I really don't get all the fuss over her.
