why are all the hard orcs coming back from the dead?
I think that's a trait. Some orcs just come back a few times upgraded. The others should stay dead.
There are also vendetta missions. Those are inherently hard, because those are the orcs that killed other players, on your friendlist or just some randoms online.
Vendetta missions are the best element of this game. Not even the nemesis system.
Decapitate them.

Nemesis literally refers to an enemy that killed you before and got promoted. Logically, if you don't die you don't get to experience how it works.
Although, there's not much to experience really. The orc gets promoted and that's it.
As I mentioned earlier the vendetta system is much better. I really like indirect online in games like that. Like players' ghosts and vagrants in Dark Souls for example. Bloodborne has those buffed enemies with glowing eyes that sometimes grab your souls when you die. But they're only the local enemies that killed you. Would be more fun if a buffed version of enemies that killed other players online could show up in your world, and you could avenge them, and get some profit out of it yourself.
I once had this one orc who would keep spawning near me every 10 minutes and attacking me. I had killed him a long time ago and he wasn't even on the army screen any more, but he just kept appearing out of nowhere no matter how many times I killed him.
The nemesis system is great but it breaks pretty easily if you never die or clear the entire army without anyone being replaced.

It is random. There's no dedicated decapitation move in the game, there are just different execution animation of the same move.
