Release was buggy as fuck, PVE experience was boring and unbalanced, the plot was/is boring as fuck, there was a lack of actual interesting content at launch, was literally barebones WoW.
Now it's free to play (but you have to pay to actually advance beyond 30), has more content, is slightly less buggy and more balanced overall, but the population has been dwindling. If they didn't release the game like they did, it could have actually been a hit. It's a cheesy and dumb Star Wars game.

Lots of things went wrong (bad/limitted engine, poor server management by Bioware, neglected PVP) - but frankly they're side issues.
The underlying problem is the core game was just not good (or fun) enough. There were some good stories in the game (Agent, Knight, etc) but certainly on release, it was an incredible chore to actually *play* the game. I could never recommend the game to anyone because despite it's merits I know that I'd be inflicting some seriously shit gameplay on whoever played it.
Shame really, there were some elements it did quite well. Huttball was awesome.

objective answer here from someone who actually cared about the game and played for a long time and still does today:
- insisted on using an engine before it was finished and ready to be used
- ignored any negative feedback no matter how it was presented
- tried to directly compete with wow from the get go
- even though they introduced f2p trying to play it completely f2p is like playing a demo, even basic shit like how many hotbars you have is extremely limited
other than that it's actually pretty fun, there's a lot of stuff that it does better than if not just as good as wow does, the only real leg up wow has over it is it's more polished and has more content that directly interacts with other players, and the pvp in wow is just overall 10 times better

I don't say that often because it's usually a very cliché word, but they just went full casual to the point it's retarded. Look nowadays : With 4.0 you could absolutely roll on everything; companion system was so god damn OP you could do H2 group quest while cooking a burger, faceroll the keyboard from time to time and still be fine. And that's without the optional second "ubergod" companion droid that turn some quest into godmode. Worst of all, when they nerfed the companion a bit so that soloing heroic quest would require a bit of skill, everybody began to whine like crybabies so much on the forums they had to buff the companions again. Now, everything solo is boring as fuck, nearly everything H2 is no challenge either, and you have to solo H4 to use some skill, but that's not always possible.
Otherwise than that, it's still deeply flawed. First of all, the combat system is utterly boring. Healer are expected to be healbots, and everything is just a copy paste of outdated MMO mechanism. They could have done so much better. I don't know, just look at games like Age of Conan. It's clunky as fuck but damn, the melee combat system is much more interesting than SWTOR crap and would be pretty sweet with laser saber. In addition, during leveling, nearly every single boss, including H4 group quest, is pretty much boring tank and spank. Even in flashpoints, it's mostly like that. You have to wait for the ops to see some propers tactics.
Second, writing is hit and miss. And no, I don't talk about retarded quest like pic related. I'm speaking about main questline. Some are actually pretty decent (Sith Warrior, Imperial Agent), but other are simply bad. Jedi Consular is atrocious, and the Bounty Huner one barely make any sense. Story feels incomplete, some part feel rushed, and writers like to kill characters (you just met 2 second ago) and try to make you care but it doesn't work. You simply don't care about them because it's badly written.

Game is full of absurd technical limitations (and it's technically sub-par anyway, just look at graphics and animations, eve considering it's a MMO), and some old as fuck bugs are still not fixed.
Game is also fucking expensive. F2P model is a joke and a shame for the industry. When they introduced it (and maybe it still the case, idk), F2P players weren't allowed to wear epic purple gear. Even if they had looted it fair and square, it was just forbidden. WHAT THE FUCK. Sure, prevent the F2P to equip the best item they loot, the best item they need for PVP and try to say it's not a pay to win, good job Bioware. Also, what? You limit their UI and don't allow them to have full access to shortcut bar? Yeah, seems fair.
So the F2P players had to camp near the market place, because some people were selling for IG credit the authorization to equip epic gear or to have full UI. But it was not always possible to buy it, especially since F2P were restricted as fuck with a stupid credit cap.

No. Seriously, if you want to be bored to death, play a tank like Powertech. Oh boy it's fucking boring. You have, like, 3 or 4 hotkey to keep on cooldown and two decent fillers. There's no interresting aggro tool except a few "ho shit" buttons that act like uber taunt of uber irritate and that immediately and magicaly give you the aggro.
Well, I'm not really honest here, they added one (1) tool recently that allow you to swap position with the current aggro holder.

They incorporated more set ups in their rotations so the combat felt engaging as far as shitty tab targetting and hotkeys goes
Here's my take on it and I played since day 1
1) Not enough end game content at launch.
You wound up playing the same shitty dungeons at end game that you grinded through to get to max level with maybe an extra "wing" or some new mobs unlocked. This is a common problem with new MMO's, they take peoples determination for granted and don't plan for those who grind all the way to max level in a week and have nothing to do but hold their dicks in their hand until the designers come up with new shit to do. If you're launching an MMO, have at least 6 months of content planned out before you open the doors, otherwise you're going to have a bad time.
- PvP was horribly unbalanced and not fun at all. The gear sucked, you died in two seconds, and those with the best *PvE* gear wound up dominating the field.
-Nothing else to do worth while besides level up other characters once you hit max level. The trading sucked, the crafting sucked, etc.
Otherwise it's a great game. The art style is great. The Jedi shit is great. Everything about it was good except for the end game. I bet that's different now but unfortunately with the way they've monetized the game and made it F2P the fucking FAQ is so god damned complicated that normal humans can't understand it. Look at this shit and tell me it's easy to understand between the different pay tiers/F2P content/etc. I dunno why this is so needlessly complex, but it is:

It's fine as an MMO but they need to get rid of all the stupid currency/subscription tiers/F2P shit.
If it's Free2Play, cap the level at 20 and give people a mini dungeon to do and the opportunity to do PvP or something. Maybe even set a time limit for 40 hours a week for a trial and that's it.
If it's subscription only, let the people do whatever they want and don't tack on some stupid unique game currency that you can get by doing shit.

This is coming from someone who Pre-Ordered the MOTHERFUCKING COLLECTOR'S EDITION OF THIS GAME and now have the box sitting on my top shelf like a paperweight.
The main problem that made me quit the game and actually begin hoping for this game's downfall was the developers CONSCIOUS FUCKING DECISION to cease all class storylines.
Now every future expansion or story that releases is just 1 story from 2 perspectives. You play the rebel side, and the empire side. There is no difference between being a BH and a SW to the story anymore aside from maybe a minor sound bite reference.
Fuck Bioware, they had a fucking amazing game that could've kept on being amazing if they were will to keep shilling out the money and expanding on it. Instead they didn't see an immediate return on their investment and went hardcore jew much to the chagrin of their fans.
Not to mention endgame at launch was pretty mediocre and so most people (myself included) got to max level and then got bored after awhile.
