I say it's bad because
Buddy's characterization doesn't make sense and it's not relatable, especially compared to Brad from the first game. Brad is trying to absolve himself from guilt through his attempts to protect or save someone close to him. Buddy is a kid who wants to kill everybody just to be powerful.
Brad's characterization during the opening scene where he forces Buddy doesn't fit at all with his desire to maintain her innocence from the first game, which was his entire motivation.
Rando being alive sucks the whole fucking impact out of Brad's actions at the end of the Painful.
It's next to impossible to beat the game without using Joy, you're pushed into it so much and there's no penalty whatsoever for repeating Brad's mistakes.

Also, Dr Yado and his shit that caused the Apocalypse or whatever is a more significant aspect of the story this time, but why? Who fucking cares? The previous two Lisa titles are largely character driven games. The subject of it isn't the world history or lore or what have you. It's the motivations, actions, feelings of the characters in it. it has nothing to do with what made the games compelling in the first place. Nothing.
Also, Buzzo showing up and saving Buddy from mutant Brad at the start was some straight bullshit that sucked the impact out of that encounter as well to try and repurpose him as a nice character. He existed to punish Brad for what happened to Lisa. Why is he all of the sudden some good guy to Buddy who gave into the darkness? What sense does that fucking make? How is that compelling? Even if he was just some hallucinogenic manifestation of Brad's guilt and self hatred, that would have been far more meaningful.

Before the kickstarter for the base game/the painful was launched, Dingaling made a small Yume Nikki-like game just under the title of LISA, where you play from the perspective of Lisa. It's not quite similar but it's good actually, has some really striking scenes. I played it back then, which is why I was so excited when The Painful got funded.

Well she's not on narcotics if you didn't use Joy, right? Then she's just a 13 year old girl but she still behaves the same way. That's the thing, whether you use Joy or not is totally meaningless, which is astounding considering how relevant it was to the destruction of Brad as well as Buddy's relationship with him.
I liked Joyful, prefer controlling one character with enemies balanced around that, than the way things were in Painful with every other enemy one shotting your party members with every attack.

Theres a party member you get that has some of the best dialgoue in the game, Nern Guan, and he tries to tell you a story of what happened, to tell you about The Flash - and he gets off track for about 15 minutes. It was a dull game sadly thanks to its genre. Painful relied on party dynamics to be interesting. Doing that same thing with only one character meant every fight could only ever be one thing and every enemy could only ever do a third of your health an attack.

However, Sticky did tell her about sex and why she's needed, so at the end of the game, she does what she needs to with the one person she can even remotely trust.Rando was Brad's adopted son. He's the one watching from the bushes in tje Painful and Buzzo is the one with the bowl cut. Buzzo saws off Rando's face in the Joyless ending's flashback, which can be seen again in the Joyful, IIRC. Buzzo went crazy after Lisa's death and used the saw to cut off Rando's face because he wanted to get back at Brad(? I forget this part). This is why Rando knows Buster Punches in the end of the Painful and uses Armstrong Style in the Joyful. The artbook also confirms his full name is Dusty Armstrong, which is a last name he adopts after essentially being Brad's son.
