The problem is that, in most Bethesda games, there's no reason not to say yes.
So, rather than try to make quest mechanics deeper and have *gasp* consequences, they decided to just cut the bullshit and have yes be the only answer.

What's most insulting of all is that even when the character would logically want to say no (or at least have the option to, like when the brotherhood tells you to wipe out the railroad) you often don't get that option.
