Some people might not find it scary but I remember when I first played Fallout 3 and got to DC I went sniping. Then since I pour into perception (because I don't like being snuck up on) I saw two reds dots in front of me. Shit terrified me as a kid. I had a thing about deep water in video games, so the entire level wad kind of a nightmare to get through, but that fucking eel was something else.

The scariest area for me remains the snork den in CoP, down in the fissures in the earth.
This creepy glitchy invincible ghost from SH4 was pretty bad on his own, but what was really spooky was if you spent too much time inside the room with the puzzle on that section, he'd warp in out of nowhere and start going wild on you.
Especially spooky as the game breaks its own self-imposed rule that you would have noticed by that point; As far as I saw, it was the only time in the game that a ghost can appear in a room that it wasn't already in. Every other ghost in the game is either ghost free, or has the ghosts in it as soon as you enter.
