With TOH, it's GOTY for me, no question. Bloodborne honestly feels like the game From always were building towards imo, calling it souls with a twist doesn't really do it justice.

The only thing about the DLC I really don't like is the massive spike in difficulty compared to the end of the game. It feels like you're stepping into a Depth 5 chalice on NG, and I can only imagine it feels worse on NG+ runs

I almost agree with this, but there are a few things I just can't wrap my head around. Blood flasks being a vastly inferior healing system compared to estus flasks, for example, and being unable to teleport from lantern to lantern. I can only imagine how awful it must have been when the loading times were longer and didn't have item descriptions to read.
I'd have liked some more large scale interconnectivity in the world, too. It was one of DaS' strengths, and the major advantage of it being open world as opposed to DeS' archstones, but now they combined the two and kind of lost what made either work.
The rest is absolutely fantastic though. It's definitely one of my favorite games.

99 percent of the people whining about the difficulty of the DLCs are whining because they're getting one-shot by enemies in NG+ and beyond.
I don't know what FROM has done with patch 1.07, but it seems as if they've revamped the damage output and health of enemies in NG+ to be much higher than it was in the previous patch.
Unless you are super, duper good at downloading enemy movesets, then I suggest you play at NG. If you don't have an end-game character ready, just fucking speed through a new character. It shouldn't take you anymore than 4-5 hours if you know what to do to get to the DLC when you start a new character.
I personally started with a new character and have only died about 3 times at the most to each boss. Other people, who started in NG+ and beyond, are reportedly dying 40-50 times on the harder DLC bosses.

It's pretty cool, but I am disappointed about a few things.

Other than those though it was pretty good. Ludwig, Maria, and Orphan of Kos are all very memorable boss fights. Laurence being a Cleric Beast reskin was kind of lame but at least he had a second form so the fight didn't feel like a complete rehash. All the new weapons are designed nicely and rather unique as well.
If you've already beaten the game it breathes enough new life into it to make you want to pick it up again.

This is pretty obviously the case. Miyazaki talked about a Beast Mode early on that would change your player and require some sort of online functionality to recover from. Made it sound like some sort of double edged sword as far as pros/cons of it. This is why people were like "where's beast mode" months after the base game released.

You only need PS+ to play (most) games online, nothing else. Restricting access to the store and services you're already paying for would be ridiculous. Just crack down and get a year membership, the free games and discounts alone are worth it.
Either way, you get a free 2 week trial if you haven't already used it, and the game should also come with a free 2 week trial as well (physical does for sure, not sure about digital), and on top of that your PS4 should come with a 30 day trial. Plenty of time to experience all the content.

How do Igit gudagainst the Orphan of Kos? His attacks all have variable lag times and hit like a truck, and most of the time I'll run out of vials/bullets half way through the fight. I've only gotten to the butterfly stage once and that was as a confederate in someone else's world.

Like AotA, the first area is a copy paste of a part of the base game with the pretext of timetravel or in this case muh dreams. You access it the same way too (need to progress to a certain point and pick up a certain item).
The best thing the DLC added was 11 new tricked weapons on top of the whooping 15 that the base game had, so it was desperately needed variety.

There is a shitty hunter's tool that allows you to heal allies with an aoe.
BB doesn't have faith utility like Dark Souls 1/2 had. DaS1 had 1 really expensive aoe heal and one really expensive heal over time in an aoe. DaS2 had all of the heal miracles heal your summon if he's close to you if I remember correctly.
Honestly, what's much more fun and practical is making a regeneration build in DeS.
