It is in my opinion that Ludwig's first form is poorly designed.
His attacks are pretty similar to some of the other beast bosses' attacks, like Watchdog's charge and Paarl's windshield wiper arms. I personally don't really like the beast bosses and find they annoying, but if you're more okay with those types of bosses, you might have a better time than me. What also doesn't help is his insanely high damage output and how quick he attacks, and it can lead to a pretty unfun trial and error experience when you get hit once and half to all of your health is gone. He also has a ton of health, so it just starts feeling kind of tedious. The camera also likes to fuck up during the fight, so that's not great.
I really enjoyed Ludwig's second form in both design and execution, though. It's much better than the first and I kind of wish it was just the whole fight instead of the half and half we got.

You're not ready. A few drop runs will net you a couple new weapons and a new armor set, but you should be ready to spend more time doing main scenario bosses and leveling until at least Bergenwerth before you try to tackle the Hunter's Nightmare.

There's two blood rocks now if you have the DLC. Rocks are pretty easy to come by if you have access to the higher-end chalice dungeons. You can just enter a blood rock dungeon glyph and get about five guaranteed rocks every playthrough assuming you decide to go through the DLC, too.

Just beat her on my second try. Ended up downing all my vials since I was getting hit by most of her stupidly long-ranged attacks. Very glad they buffed Augur, it's proving to be pretty useful against humanoid opponents.
Really enjoying it so far. Just got to the fishing village and I've already had two boss fights where I ran out of vials and practically had a heart attack when I finally came out on top. Not felt like this since I beat Kalameet.
Orphan is a badass fight, very reminiscent of Manus. Pushing shit in hard core though, rough.

His first phase is very predictable. You can beat it without getting hit at all and I know for a fact that at NG++ with 40vit only full hits one shots, you can still heal through partial hits.
Second phase is more bullshit because he doesn't stop attacking so dodging is mostly waste of time. Just unlock, hug his legs and spam R1 until you can do viceral attack. Heal when he try to do the AOE thing and when he does the whole room aoe just go behind him and keep spamming R1. He's easy to stagger for some reason.

Ludwig is by far the hardest boss of the DLC so it's a bit silly that they put him first. I died to him about 30 times before I finally figured out all his attacks and then died once to the aliens and three times to Orphan.

Ludwigs channeling energy shit killed me at full health, the shark fucks have a weird lunge that just wrecks your ass since it hits you like 4 times, The living failures are extremely easy till they do their weird spell with meteors that two shot you while stunning you, Maria was extremely fair but Orphans just a motherfucker

The first form is definitely a lot harder. His left side is always safer than his right. When he jumps up onto the ceiling his fall is based on an audio cue, the only way to dodge it is when he roars again. That's all the general advice I can offer.

I literally started NG+ yesterday morning
I ran all the way to insight by the 3 crows from the sewer
got back, leveled up, beat Cleric first try, ran to gascoigne, cheesed him on the stairs, beat him 1st try then took a good 7 hours trying to do everything in the cathedral ward and beat vicar amelia
it didn't take too long in comparison to when I first played mainly because I knew everything already and how to beat them
I beat her just in time for the midnight release for the dlc item

I'm not sure that BB nightmares work that way. Seems like the only things that can imagine things to existence in the Nightmares are the Great Ones who source them (MP, Mergo, Kos, etc.), or the hosts of the Nightmare (Gehrman who CEd his workshop into the Hunter's Dream, Micolash, Maria, etc.)

Body of Kos washed ashore a fishing village, with the Orphan of Kos incubating inside of it. The locals found the parasites, and stared mutating themselves with it.
Byrgenwerth came along, and started doing gruesome "science" on the fishpeople, and experimenting on Kos, which eventually produced Rom.
The Orphan of Kos, being a Great One, and therefore sympathetic, grew tired of the College abusing it's people. It sucked the whole village into the Nightmare, and the College couldn't follow because Rom was cockblocking them.
Years later, the Choir found a back entrance into the Nightmare through Rom's obstructions, and set up research labs. They eventually re-discovered the fishing village as well.
One of their chief researchers was Maria, but she eventually realized the atrocities that she was committing, and regretted her choices. She tossed her weapons down a well in disgust, and sought to atone for her sins.
To do so, she became the Host of the Hunter's Nightmare in a pact that was similar to Gehrman's, and pulled a bunch of blood-drunk hunters into the Nightmare with her, as well as ensuring that all future blood-drunk hunters would join the fun as well. She also tried to kill herself, but that doesn't work when you're the host of a nightmare -- just as Gehrman how easy dying is.
The hunters BTFO the local Choir staff, and their ranks formed an impenetrable barrier against any people who might want to come knocking. Thus was the safety of the victims of the Church's experiments ensured.
You find the fishing village after fighting your way through the Hunter's Nightmare, and kill the Orphan. It's finally able to let go of it's anger, and it releases the curse that it had put on the Old Blood (the Vermin, and therefore Beasthood, which it had implemented as revenge for Byrgenwerth's atrocities), and returned to the ocean
