They don't even do real piledrivers anymore, Tombstone piledrivers are much more safe as it doesn't involve the risk of the other guys head ever reaching the mat. Top rope stuff is much more dangerious by and large.
Real piledrivers were banned after the Owen Hart-Steve Austin match where Austins neck was broken by a botched piledriver, To give credit to Austin, he finished the damn match, it wasn't pretty though.
