Since no one else has said anything yet, melee combat is completely unreliable beyond easy difficulty. It's meant to be used at a higher level with perks like ninja, sandman and sneak skills. If you're expecting to just meat head bum rush the raiders you're gonna get fucked.
Unarmed is even worse than that because there's hardly any modifiers or skills books out there to help you.
