The issue come from the brain developing as a brain of the opposite gender than the rest of the body, and therefore, not reacting well to the hormones feedback coming from the gendered endocrines gland.
Most of the time it result with depression and an unease with the body, it often lead to the person killing themselves if not addressed. The treatment is usually hormonal therapy, which balance back the Hormones in the body to what the brain, is actually expecting, but it also result with altering the body, affecting the genital and rendering them useless.
The surgery is mostly aesthetic (the real treatment are the Hormones), but address the change caused by by the hormonal therapy and provide a bady with which the patient is comfortable with.

The "regret" mostly come from people for which the operation is not enough.
The suicide rate hate pre-op for Trans is around 41%, post op, it get reduced to 4.1% It still quite higher than the average population suicide rate (1.6%), but it's an undeniable improvement compared to Pre-op suicide rate.
Transgender operation actually reduce suicide rate. It's just that the operation is in no way the perfect solution.
