Either being a RPG, or having any semblance of gameplay if you decide to dumb down the RPG elements.
Having neither, like 3 and NV has, is pretty pointless. Decide on one direction (that is not "casual shit for babies"), and exploit that to its fullest.
Modern fallout games so far have been the FFXIII of WRPGs. Stripped down rpg mechanics, yet no action engine (or anything else, really) added to make up for lack of the former.
I like videogames, not soulless minimum common denominator frankensteins, they gotta have at least something of high quality in there. If a 20 years older game outdoes you in everything you have, that should be alarming.

I believe they have restored roads in their core region, a decent railroad network and I believe 1 in 200 citizens has a car. Its just a bit hard to realize just how massive the NCR has become given that we only see them on their fringes.

New Vegas is no doubt a better game, but I still got a lot of enjoyment out of FO3 (and skyrim for that matter).
All of bethesda's games (even morrowind) have glaring flaws, but I always get at least 100 hours out of each game despite them. Especially with the modding community.
Say what you want about the quality of Bethesda's games, but their business practices are top-tier. No modern developer comes anywhere close to supporting the modding community as bethesda does. Also all their games have no DRM, no day 1 DLC, and the DLC they do eventually make are usually 10 hours worth of gameplay content.
Hopefully they can do enough bugtesting before the game launches this time around.
