I never noticed how detailed the reloads are in this game. There are empty and loaded magazine models and if you can see the breech of the weapon you can actually see the round being chambered (most noticeable on the bolt action rifle reloads).

If you look when it's empty there's a small button the side that pokes out. That's the bolt release. Pushing that will release the bolt to send the first round of the new magazine into the chamber.

When your gun runs dry and you reload, there's no bullets in the chamber, because you already wasted all your amunition. You pull the charging handle, nothing comes out, but you're doing it to load a new round into the chamber.
Guns only eject rounds from the chamber when you pull the charging handle without having used up all your ammo. That's where tactical reloads and the 30 +1 gimmick comes in, but that's another story.

They should be pretty expensive and have good quality, since they're like, you know, what grants you the ability to shoot more than one bullet at a time without having to manually reload one into the chamber after each shot.
You only throw them away during emergency reloads, when they're devoid of ammo and you're in a combat situation. If it has no bullets, throw it away, load a new one, deal with the threat, then come back and retrieve it from the ground when you're done.
